Transition to Adult Healthcare

Transition planning is often associated with education, employment, and life after high school—but healthcare transition is equally important. As youth with autism spectrum disorder and related disabilities move into adulthood, they also transition from pediatric to adult healthcare systems, requiring new skills, increased independence, and coordinated support.

This Florida Atlantic CARD training will provide an overview of healthcare transition, including when transition planning should begin, barriers that may impact successful transition, and strategies to support individuals and families throughout the process. Participants will also learn how healthcare transition can be incorporated into IEP transition planning and how educational and healthcare systems overlap during the transition to adulthood.

Topics include: • What is healthcare transition • Pediatric vs. adult healthcare systems • IEP transition goals and healthcare independence • Postsecondary transition considerations • Common barriers and challenges • Self-advocacy and independence skills • Tools and resources for families and professionals
